Methodology

The math behind the model.

Every figure on this page comes from a formula, not a guess. Here's how each one is built so you can check our work.

01 · NCR

Net Collection Rate

NCR = payments collected ÷ net charges, where net charges are gross charges minus contractual adjustments. We subtract revenue abandoned on unworked denials to show your current effective NCR. A healthy practice holds 95% or better.

02 · REWORK

Rework cost

Annual rework = denied claims × share appealed × cost per claim × 12. The default $57 per claim is the MGMA national average; switch the selector for in-house, specialty surgical or high-complexity hospital settings.

03 · A/R

A/R depreciation

Each aging bucket is discounted by its historical collectibility: 95% for 0–30 days, 85% for 31–60, 65% for 61–90 and 30% for 91+. The result is the realizable value of your ledger, not its face value.

FAQ

Questions we hear before every audit.

Short answers to the numbers most practice managers check first.

What is a good net collection rate?

95% or higher is the standard for a healthy practice. Below 90% usually points to denial or coding problems that compound over time.

What denial rate should I aim for?

Best-in-class practices hold initial denial rates between 5% and 10%. At 12% or above, rework cost starts eating margin faster than most practices realize.

How much does it cost to rework a denied claim?

MGMA puts the national average around $57 per claim. Specialty surgical appeals and high-complexity hospital claims run higher — $85 to $118 in our model.

What is a healthy days sales outstanding?

30 to 40 days is typical. Above 45 days means cash is sitting in A/R that should already be in the bank.

How much of 90+ day A/R is actually collectible?

In our model, about 30%. Timely filing windows and payer policy close off the rest. That's why oldest-first work matters.
